Athletic Eligibility Packet

Palm Beach Medical Insurance Liability Form - Form is used from June 1 to July 27, 2026. Athletic Insurance does not cover Open Gyms or Camps

Steps to Complete the Athletic Eligibility Packet via Aktivate

  1. Registration for Parents: 
    1. Go to aktivate.com 
    2. Click Login 
    3. Click Create an Account (You only need ONE account, even if you have children in more than one high school and/or junior high; Do Not create another account if you have used Register My Athlete in the past)
    4. Fill in personal account information (this should be the Parent’s personal information) 
    5. You will be using the site as a Parent
    6. Click Submit 
       
  2. After you have an account:
    1. Login 
    2. Under the Parents header (Blue), Click the button labeled “Click here to start/complete athlete registrations “. (The first time you log in you will be asked to agree to terms and conditions) 
    3. Click Start/Complete a Registration (upper left-hand corner of the page) 
    4. Click Start a New Registration - this is where you will enter all your Athlete’s information
    5. Click on the red bars to complete all requirements 
    6. Click the orange button on the lower left side of the screen for live chat or email support@aktivate.com for assistance
       
  3. Complete and Upload the Following Documents to Aktivate:

PBCSD 1588 - Athletic Eligibility for High School Students

  • Complete Proof of Insurance, if none write “no insurance”
  • 10 initials, notarized
  • Student & Parent signature, notarized
  • Upload all four pages

PBCSD 1589 - Student Medical Consent for Athletics

  • Complete emergency phone number
  • Student & Parent signature, notarized

PBCSD 2608 - Interscholastic Athletics Accident Insurance

  • Parent signature
  • Payment made on schoocashonline.com or check made out to DJGHS turned in to the Athletic Office
  • Receipt of payment uploaded with form  

FHSAA EL1 - Electrocardiogram Screening
New form for incoming Freshmen and New Athletes only

  • Parent signature
  • Physician Signature
  • Attachment validating clearance uploaded to Aktivate

FHSAA EL2 - Preparticipation Physical Evaluation
*NO SUBSTITUTIONS, GOOD FOR 365 DAYS FROM DATE*

  • Upload page 4, Medical Eligibility - Student & Parent signature, must be stamped by a medical practitioner

FHSAA EL3 - Consent and Release from Liability Certificate

  • Pages 1 - 5, student and parent signature
  • Upload all five pages

NFHS Videos (4) - Must print and include in packet

  • Concussion in Sports
  • Heat Illness Prevention
  • Sudden Cardiac Arrest
  • Sportsmanship
  • Must be in the name of the student athlete
  • Must be completed for the current school year (dated after June 1, 2026)
